Health coaches and health improvement practitioners work with you to improve your recovery or overall health and wellbeing. You can access both here at Woodham Road Medical, simply ask the GP or nurse during your appointment or inquire at reception. This is a FREE service to you.
Daisy Sare
Health Coach
As a health coach, Daisy supports patients to make lifestyle changes that improve their health and wellbeing.
From providing self-management support, through providing relevant information, health management skills, promoting behaviour change and working with patients to create smart goals/action plans.
Daisy can support with:
- Exercise
- Nutrition/eating
- Weight management
- Sleep
- Smoking cessation
- Managing long term conditions such as: diabetes, cholesterol, gout and high blood pressure.
- Connection to relevant community services.
Daisy also, does home visit appointments, recreational and supporting patients to hospital appointments, and GP or Winz appointments on Wednesdays from 9am-12pm.
Walking + coffee group on Fridays from 9:30am- 11am. Please let the reception, GP or nurse know if interested.
Rowena Smithies
Health Improvement Practitioner
Health Improvement Practitioners are qualified health professionals, with experience in physical and mental health, who have completed specialist training in the Te Tumu Waiora (to head towards wellness, access and choice) primary mental health model.
Rowie is a registered nurse with previous experience in mental health, practice nursing, palliative care and Brief Intervention.

As a practice nurse, I often found myself spending time with people who felt overwhelmed and a bit lost so was inspired when the HIP role became available. What’s important to me is being part of a holistic primary health care team. I work closely with the whole practice including our Health Coach to ensure a tangata whaiora (person seeking wellbeing) is thoroughly heard and empowered.
I am a FREE service and you can come and see me as many times as you like. You do not need to see the GP first.
Heads up, I am not a counsellor or therapist. I am a professional that supports all registered people at Woodham Road. I have with some good ideas to enhance wellbeing at all ages and stages. I can support you to make a behavioural change plan based on the issue that is most important to you at the time of being seen.
I work Monday, Tuesdays and Wednesday and often can be seen on the day.
